New Service Request

Welcome to the Booneville Water Department Community.  For new service, we ask you to stop by our office to fill out a small application. For faster service, you can print the service request form below and just bring it with you.  We are located at 232 N Kennedy Ave. You will also need to bring a valid drivers license and your deposit for new services.
